Formoso (Hendrickson) of McPherson, Kansas.; his wife Melissa Pritchett; his daughter Brittany Smith (Keith) of Fort Necessity; his stepdaughter Kristen Maestas (Christine) of Seagoville, Texas; and his brother Manuel Formoso (Denise) of Commerce. He is also survived by his 4 grandchildren.
