He is survived by: his parents, Brian Forck and Lisa Forck; his siblings, Chase, Kaelin, Chelsi and Jaemin Forck; and his grandparents, Steve Hencey, Marilyn, Joseph Holtmeyer, Carolyn and Dennis Forck. He is also survived by his many aunts, uncles, and cousins.
Memorials are suggested to the American Heart Association and the Make-a-Wish Foundation in honor of Cole.
