He is survived by: his children, Matt Cook (Jill) of Mulvane, KS, Stacey Woods (Barry) of Meade, KS and Mary Packard (Rob) of Plains, KS; his siblings, Raymond Cook of Midland, TX and Debbie Avila (Robert) of Haysville, KS; and his sister-in-law Donita Box (David) of Liberal, KS. He is also survived by 10 Grandchildren and 3 Great Grandchildren.
