He was predeceased by: his parents, Gayle Eugene Cook and Rose Marie Newland Cook; and his brother Charles Robert Cook. He is survived by: his wife Amy Sue Olson; his children, Samantha Sue Cook Chong (MunLai) of Gardner, KS, Eric Eugene Cook of Lawrence, KS, David Samuel Cook (Martika) of Kansas City, KS, Isaac Benjamin Cook of Lawrence, KS and Maxwell Paul Cook of Lawrence, KS.; his siblings, Gaylene Rose Tunison and Theodore Ray Cook, MD (Karolyn, MD); and his grandchildren, Elijah Yi Chong and Charlotte Xuan Ruo Chong.
