He is survived by: his wife Teresa Anzo; his daughters, Ashley of Springfield, MO and Amber Foulk of Springfield, MO; his stepchildren, Tiffany DeBerry (Ryan) and Tim Dowsey (Kerry); his brother Gregg Foulk (Terri); his grandchildren, Graysen, Kaedance, Kohan, Kahne and Gannon; his stepgrandchildren, Peyton, Wyatt, Brecken, Lawson, Stella and Annsley; his niece London Foulk; and his nephew Garrison Foulk.
