He was predeceased by : his parents, Benjamin Leslie Sample and Dorothy Sample (Epler); and his siblings, Lyle Sample, Leon Sample and Doris Jean Rachau.

He is survived by : his wife Eileen Sample; his daughters, Deb Micek (Tim) and Dianne Wyrick (Mike); his granddaughters, Jenna Wyrick (Tyler Samuelson) and Kayla Wyrick; his sister Donna McClain; and his sister-in-law Barb Sample. He is also survived by numerous nieces and nephews, and a host of friends and relatives.
