He was predeceased by: his parents, Francis R. Sr. Applegate and Charlieann Applegate; his brothers, Rodney and Evan Applegate; and his children, Darin Applegate, Kirsten Daily and Noelle Fox.

He is survived by: his son Haven Applegate of Hays; his grandson Troy Applegate (Connie) of Celina; and his brother Alan Applegate of Roswell.

Memorials are suggested in Dr. Applegate's memory to the Dean McGee Eye Institute in Oklahoma City.
