She was predeceased by: her parents, Gerald Wayne and Sally Bella Ochs (Diaz); her parents-in-law, Doris and Matthew Smolik; her granddaughter Aliayah Carter; and her nephews, Chris Smolik and Michael Cox.
She is survived by: her grandson Ladonte; her husband Steven R. Smolik of Great Bend, Kansas; her children, Jonathan Velasquez (Trish) of Salina, Justin Zadoka of Great Bend, Steven R. Velasquez (Abby) of Salina, Kansas and Tashina M. Diaz (Michael Carter) of Wichita, Kansas; her sister Kathleen Franzen (Eric) of Great Bend, Kansas; and her great-uncle Robert Ochs of Hayward, California. She is also survived by many nieces and nephews.
