He was predeceased by: his father Charlie "Choc" Perkins; his father-in-law Joe Lee Wright; and his sister-in-law Jan Perkins. He is survived by: his mother Elda Perkins (Janzen) of Lorraine; his wife Leslie Perkins; his sons, Levi Perkins (Jenifer) of Topeka and Aaron Perkins of Wichita Falls; his siblings, Herbert Perkins of Lorraine and Cindy Perkins-Wereley (Brian) of San Antonio, Tex.; and his grandchildren, Joe Perkins, John Perkins, Matthew Perkins and Lily Perkins.
In lieu of flowers memorials have been designated to Central Kansas Dream Center, in care of Bryant Funeral Home.
