He was predeceased by: his parents, Guy Leslie and Neva Church (Stowe); and his sister-in-law Carol Church. He is survived by: his siblings, Earl Church, Orville Church (Mary Jane) and Sue Church (Gary). He is also survived by three nephews and one niece.
