She was predeceased by: her mother Velma Hayes (Bohnsack); her uncle Carl Bohnsack; and her grandparents, Ina, Kenneth Bohnsack, Martin and Alice Hayes.
She is survived by: her husband Tom; her children, Bridget, Jack and Liam; and her father Larry Hayes (Roberta).
