He was predeceased by: his parents, Leo F Forge and Irene Culp Forge; and his brother Raymond Forge. He is survived by: his siblings, Walter Forge of Leavenworth and Margarete Kowalewski (Joe) of Clinton, Tenn.; his nephew Brian Kowalewski (Tina) of Fredericksburg; and his nieces, Marilyn Jones (Ken) of Lenoir City, Shawna Herd (Keith) of Pontiac and Nancy Forge of Leavenworth.
