He was predeceased by: his parents, Thomas Ballard and Prudence "Minnie" Ballard (Castle); and his siblings, Tom Ballard and Betty Mayfield. He is survived by: his wife Pamela Ballard; his daughters, Angela MacGowan (Buck) of Oklahoma City and Alyssa Milstead (Blake) of Lockney; his grandchildren, Chase MacGowan, Kaelynn and Kinsley Milstead; his siblings, Richard Ballard of Palm Springs, Denny Ballard of Santee and Dawna Wagner of Columbus; and his pets, Milo and Bella. He is also survived by Numerous nieces and nephews.
