He was predeceased by: his parents, Leo Cook and Elsie Cook (Quillen); his wife Sue Cook (Cash); and his siblings, David Cook, Jerry Cook and Shirley Accord.
He is survived by: his children, Shane Cook (Kelly) of Wichita and Ronna Fulkerson (Rick) of Mulvane; and his grandchildren, Sam Razey (Mikali), Missy Moore (Ricky Ruff), Jerry Fulkerson (Kayla), James Fulkerson (Hailey), Jenni Fulkerson and Jackie Fritsch (Anthony). He is also survived by 12 Great grandchildren and numerous nieces and nephews.
