He was predeceased by: his parents, Bert Matlock and Grace Matlock; his wife Evelyn "Bette" Venus; his stepson Randy Simpson; and his siblings, Faye Pens, Leige Matlock and Herbert Matlock.
He is survived by: his stepdaughter Tisha Bejanpour. He is also survived by six grandchildren, and five great grandchildren and numerous nieces and nephews.
Memorial contributions can be made in Forest's name to Harry Hynes Memorial Hospice.
