He is survived by: his parents, Judy Smith and Terry D. Smith of Augusta, KS.; his close friend Terri Blackston; his children, Zackery Smith (Rachel WidtFeldt) of Omaha, NE, Madison Smith of Wichita, KS and Evie Blackston; his brother Jason Smith (Rosita); and his nephews, Brandon, Tyler, Dominic and Oliver Smith.

Face masks and social distancing will be required.
