He was predeceased by: his parents, William Cook and Thelma Cook (Thomas); and his siblings, Doyle Cook and Hilda Schmidt. He is survived by: his wife Della Willene "Willie" Hendrix; his children, Corky Cook (Tamara) of Augusta, Jamie Ketley (Ben) of Augusta, Traci Hayden (Kevin) of Augusta and Brent Cook of Augusta; his siblings, Glenn Cook (Lucy) of Wewoka, OK, Wanda Hensley of Wewoka, OK and Shirley Clark (Preston) of Cleveland, OK; and his sister-in-law Patsy Cook of Wewoka, OK.. He is also survived by 8 grandchildren and 18 great-grandchildren.
Memorial Contributions be made out to Lincoln Elementary School or Alzheimer's Association of Wichita, left in care of the Dunsford Funeral Home.
