He was predeceased by: his parents, Hugh Northwest and Maude Forsythe Northwest (West) of Fredonia, Kansas.; and his siblings, Herbert Forsythe, Clayton Forsythe, Bill Forsythe, Max Forsythe and Martha Meriwether (Forsythe).
He is survived by: his wife Dorothy Gentry; his son Michael Forsythe (Mary Ann) of Frontenac; and his grandchildren, Megan Hague (Doug) and Garrett Forsythe (Lindsey). He is also survived by 6 great-grandchildren and many nieces and nephews.
