She was predeceased by: her parents, Johanne and J. Ely of New Augusta, Indiana.; her daughter Erin; and her friend Meg Emerson. She is survived by: her husband Jim Carr; her brothers, Ralph and Jim Ely; her friends, Roberta Witchger, Phyllis Bishop, Kim Harper and Valerie Votgner; and her caregiver Erin.
In lieu of flowers, Susan requests that all support music, arts and theater in our schools. A special fund in honor of Susan has been established for donations to: Eastwood Middle School Choir Fund.
