He was predeceased by: his mother Mildred Forgey; and his grandparents, Letha, Osby Thieke, Osa and Earl Forgey.

He is survived by: his wife Katherine; his father Dale Forgey; his sister Susan Perry (Carl); his nephew Tyler Perry; his brother John Cox of Logansport; and his grandchildren, Mariah Moore, Kendrix Krock, Viktor, Xander Forgey, Hunter, Addilyn and Korbin Forgey. He is also survived by his four children.
