He was predeceased by: his parents, Joe Cook and Elberta Cook (Gilmore); and his sisters, Patricia Joan Fitch and Sharon Elberta Davis. He is survived by: his wife Jacque (Bordner); his children, Stan Cook (Holly) of Wolcott, Mike Cook (Jan) of Mariemont, OH, Laura Black (David) of Woodstock, GA and Annette Suchanek (David) of Villa Hills, KY.; his grandchildren, Alyssa Friesen (Jacob), Ben Cook (Hannah), Bryce Cook (Marissa), Samantha Cook, Josh Cook, Mandy DeSantis (Colby), Aaron, Black, Lyndsay Suchanek and Lauren Suchanek (Austin Suchanek); and his great grandchild Iris Friesen.
Memorial contributions can be made to the "Jerry Cook Scholarship Fund" in care of Bank of Wolcott - a one-time scholarship to a local student in Jerry's honor.
