He was predeceased by: his grandparents, Jack, Doris Bush, Richard and Mary Phillips. He is survived by: his parents, Shawn Bush of Farmland and Ed "Stimey" Phillips (Nate) of Swayzee, IN; his brother Garrett "Tink" Page of Ridgeville; his girlfriend Melody Gullett of Muncie; and his grandson Camden. He is also survived by 1 niece, 3 nephews, as well as aunts, uncles and cousins.
