He was predeceased by: his grandparents, Rita Bash, Dorothy and Dalton Dalzel; and his uncle Jay Jordan. He is survived by: his daughters, Jerricah Mayfield and Kylie Thornburg; his mother Sandra Dalzell (Ralph); his siblings, Carla Guthrie (Matthew), Moe Dalzell (Mika) and Patricia Jeffries (Joe); his grandchildren, Jaelyn Beane, Jordyn Beane, Drake Croucher, Daxtyn Croucher, Parker Mayfield and Cooper Mayfield; his grandfather Jim Jordan; and his close friend Missy Martin. He is also survived by aunts, uncles, nephews, nieces, and great nieces and nephews.
