He was predeceased by: his parents, Bernard Kuzma and Denise Kuzma (Bente); and his brothers, Bernard, Michael and Steve Kuzma.

He is survived by: his wife Maria; his daughters, Karen Kuzma, Julie Kuzma (Jonathan Bailon) and Diana Trevino; and his grandchildren, Jacob Kuzma, Sophia Bailon and JJ Bailon.

In lieu of flowers, memorials to the Whiting Gimme Shelter (for pets), P.O. Box 591, Whiting, Indiana 46394, would be appreciated.
