She was predeceased by: her parents, William Surface and Mariam Surface (Everson); and her husband Mike Goforth. She is survived by: her children, Tracy Selke (Larry) of Indianapolis, IN and Jeff Goforth (Lisa) of Tulsa, OK; her grandchildren, Phil, Jon, Rachel, Zach, Jonathan and Kate; her great grandchildren, Sailor, Stella and Andrew; and her sister Carolyn Snyder (Bob) of Crawfordsville, IN.
To help prevent the spread of coronavirus, the family requires that all attendees wear face masks, social distance during the service, and avoid physical contact.
