He was predeceased by: his parents, Richard Dean Hayworth and Helen Joan Hayworth; his brother Rick Hayworth; and his mother-in-law Marilyn Atkinson Timmons. He is survived by: his former wife Deborah Atkinson; his wife Deb; his daughters, Erin Mills (Andy), Jill Keys (Kody), Sara Choplick (Ian) and Jenna Matatall (Mike); his grandchildren, Olivia, Rhett Mills, Kyan, Kaden, Adelynn, Arden, Benson Keys, Noah and Elleigh Matatall; his sister Roxanne Ewing (Charlie); and his father-in-law Richard Atkinson.
