He was predeceased by: his parents, Malvin Cook and Rosie Dell Cook (Smith); and his daughters, Patricia and Mary E. Evans (Carl). He is survived by: his sons, Clarence E. "Jack" Cook (Marsha) and William M. "Bill" Cook (Doris); his grandchildren, Matthew Cook (Janet), Leticia Burns (Byron), Brian Cook (Karin), Erin Saunders (Travis), Isaac Evans (JaKayla), Philip Evans (Camille), Joy Green (Evans) and Marco Barnes (Tashanna); his great grandchildren, Jonathan, Alyssa, Jada Burns, Brenden, Sydney Cook, Malachi, Caleb, Daniel, Ethan, Averle, Silas Evans, Xavier and Sasha Saunders; and also Jerry Yoder. He is also survived by nieces, nephews and cousins.
