He is survived by: his father Jon D. Kollada Sr. of South Bend; his sons, Garrett and Anthony; his mother Carmen Erickson; his sister Christine Kollada of Reynolds; and his stepbrothers, Edward Erickson and Nicholas Erickson of Denver, CO.. He is also survived by many loving aunts, uncles, nieces and nephews, and many loving cousins.
