He was predeceased by: his father John Anthony Flak (Pittman); and his nephew James Flak. He is survived by: his mother Jean Flak; his siblings, Ann Fischer, John Flak (Jenny Burkholder), Anthony Flak (Melissa) and Joseph Flak (Melisa); his nieces and nephews, Tayler, Devon Fischer, Dylan, Harper Flak, Colleen, Justin, Elijah (Sabrina), Lacy, Levi Flak, Barbara Jo and Jacob Flak (Brooke); and his great-nieces and great-nephews, Hayden, Sophia, Nova, Senna Flak and Gavin Crone.
