He was predeceased by: his parents, Alois Hoffman and Alma Hoffman. He is survived by: his wife Mary Hoffman; his daughters, Jen Lyness (Jason) of Newport, KY, Sara Wert (Larry) of St. Leon, IN and Bridget Allen (Don) of St. Leon, IN.; his grandchildren, Lexi Wert and Peyton Wert; and his siblings, Daniel Hoffman (Debbie) of St. Leon and Sandy Eckstein (Rodney) of St. Leon.
Memorial donations can be made to Milan Athletics in honor of his grandchildren who play sports for Milan High School.
