She was predeceased by: her parents, Joseph A. Mastalerz Jr. and Evelyn C. Mastalerz; and her brother Joseph E. Mastalerz. She is survived by: her husband Paul Kenneth Fox; her children, Matthew Fox (Helina) and Amanda Shotts (Bryan); her grandchildren, Victoria Shotts, Jensen Shotts and Emmett Fox; and her siblings, Mary Anna Elias, Judith Guilliam, Susan Jackson, Linda Bell (Dave), Paul Masters (Sherri) and Eddie Mastalerz (Lesley). She is also survived by many cousins, nieces and nephews.

In lieu of flowers, Donna requested that memorial contributions be made to the Indiana Chapter of the Juvenile Diabetes Research Foundation.
