He is survived by: his daughter Frances Jean Melvin; his parents, Tom Melvin and Judy Melvin; his sisters, Amanda Faulkner (Alan) and Maria Melvin; his former wife Christy Cressman; his nieces and nephews, Adam, Alexa Faulkner and Raelee Melvin; and his aunts and uncles, Bo Melvin (Terry), Lynn Adams (Keith), Kathy Ader (Jim), Joe Dickey and John Lennie (Pam).

Contributions may be made to the Education of Frances Melvin.
