He is survived by: his children, Steven Thomas Hook Jr. and Autumn Welker (Norm); his caregiver Cheri Rice Thomas of South Bend; his siblings, Jackie Clark of Lafayette, Kay Culp of Mishawaka and Jim Hook of Edwardsburg; his nephew John Hook; and his grandchildren, Alex Welker, Joe Bucher, Jeanavieve Bucher and Cole Hook.
