He is survived by: his wife Diane (Zarzyka); his children, Justin Forsythe (Jitthida) of Chicago, Cory Forsythe (Lindsey) of Louisville, Kentucky and Danielle Forsythe of Bremen, Indiana; and his grandchildren, Aiden, Kitana, Katalia, Kessara and Harlow.
