He was predeceased by: his parents, Joseph O'Chap and Bernice O'Chap (Lagocki); his former wife Lucille O'Chap (Sobieralski); his close friend Delphine Mutchler; his brother Eugene O'Chap; his stepson Stephen David Shields; and also, Marjorie O'Chap (Shields) and Christine Kaczmierzak.
He is survived by: his children, Maxine O'Chap (Karen), Dale O'Chap (Julie) and Kevin O'Chap (Stan); his grandchildren, Ana Everett (Jeff), Monica Foley (Tim) and Bradley O'Chap (Ashley); and his great grandchildren, Reagan, Ethan Everett and Olivia O'Chap.
