He was predeceased by: his parents, Elmer Cooksey and Kathleen Cooksey (Campbell). He is survived by: his children, Ryan Cooksey, Josh Cooksey and Ashley Hasler (Jack); his sisters, Connie Frye (Dave) and Kathy Thompson; his nephews, Brian Thompson and Brad Thompson; and his niece Christine Strauser.

In lieu of flowers the family wishes memorial donations be made to the Riley Children's Hospital.
