He was predeceased by: his parents, Charles Nolan Cook and Imogene Mart (Hunt); his stepfather Hanson Mart; and his brother Donnie Cook. He is survived by: his wife Barbara Cook (Johnson); his sons, Tommy Ray Cook (Robin) of Sandborn and Kevin Dale Cook (Becky) of Sandborn; his grandchildren, Clay Cook (Jordy) of Sandborn, Sarah Driver (Reis) of Paducah, KY, Kaitlyn of Evansville and Ashley Cook of Evansville; his great grandchild Lilly Mae; and his siblings, Darrell Cook (Molly) of Nortonville, KY, Ray Mart of Crofton, KY, Carolyn Hammell of Tampa, FL and Loretta Owens (Dennis) of Crofton, KY..
