He was predeceased by: his father Roger Fultz. He is survived by: his mother Joyce Tompkins Fultz of Salem; his wife Anna Briscoe Fultz; his stepchildren, Brett Cornwell of San Antonio, Kasey Cornwell of Indianapolis and Miriah Zink of Salem; his brothers, Tom D. Fultz (Roxanne) of Salem and Chad E. Fultz (Pam) of Salem; and his nieces, Shekinah Lincks of Salem and Hannah Cooper of Sellersburg. He is also survived by nieces and nephews.
