He was predeceased by: his parents, Everett Earl and Wilma Hedrick Qualkinbush; and his daughter Melissa Badger. He is survived by: his wife Karen Chastain Qualkinbush; his children, Trent Qualkinbush (Debbie) of Fountaintown, Meleah Johnson (Jimmy) of Campbellsburg, Yvette Persinger (Gary) of Salem and Christina Worley of Campbellsburg; his son-in-law Lee Badger of Campbellsburg; and his sister Helen Chastain of Salem. He is also survived by 14 grandchildren and 1 great-granddaughter.

In lieu of flowers, the family asks that donations be made to Claysville Cemetery.
