He is survived by: his mother Lisa Lewis (Cullins); his children, Colton, Connor, Laiken Michael Lewis of Salem and Kattie Perkins; his stepparents, Lisa Wynn (Michael Maness) of Salem and Michael Lewis (Phyllis) of Jeffersonville, Indiana; his cousin Angie Floyd of Salem; and his fiancée Rachel Gibbs of Louisville, Kentucky.. He is also survived by several aunts, uncles, nieces and nephews.
