He was predeceased by: his parents, Owen Cook and Sarah Cook (Prather); and his siblings, Harlan Cook (Rosalie), Earl Cook (Reba), James "Bud" Cook (Charlene), Gilbert Cook, Mary Beard (Joe) and Hazel Coyle (Carmen).
He is survived by: his wife Sue Dunn; his children, Jeff Cook (Kim) and Lori Hill (Brian); his grandchildren, Conner Cook (Jasmine), Sarah Ray Cook, Meghan Reed, Grant and Abby Hill; his great grandchild Presley Marie Cook; his sister Joann Crawford; and his sister-in-law Helen Cook Smith.
