She was predeceased by: her parents, Fred Roehm and Clara Roehm; her husband Ray Forman; and her daughter Clara Melvin. She is survived by: her sons, Ray Forman (Helen) of Rushville and Fred Forman of Rushville. She is also survived by 5 grandchildren, 7 great-grandchildren and 1 great great- grandchild.
