She was predeceased by: her parents, Myrtle A. Thompson (Jacobs) and Charles D. Thompson. She is survived by: her son Raymond Forman (Samantha) of Shelbyville, IN; her grandchildren, Brieann Easterday (Jonathan), Dylan Forman and Brittany Higdon (Zachary); her great grandchildren, Drelinn, Xavion, Zachary and Liam; and her siblings, Suzie Thompson, Shirley Glover and Butch Thompson.
