She is survived by: her husband Jeff Simons of Rockport, IN; her children, Shari Percifield (Mike) of Rockport, IN and Wesley Horn of Rockport, IN; her grandchildren, Levi Percifield (Kirstin), Bailey Percifield, Bre'Kara Horn, Alyvia Horn and Savannah Horn; her great grandchild Lola Percifield; and her stepmother Dorothy Beckort.
