He is survived by: his father Roy Arthur Perry; his wife Abby Perry of Plainfield; his children, Kaden Perry (Lilly) of Martinsville, Kalija Perry, Logan Perry and Sidney Perry of Plainfield; his mother Loretta Perry (Thompson); and his sister Kristen Smith of Martinsville.

In lieu of flowers a fund will be established to benefit his children.
