He was predeceased by: his parents, Harold Lee and Phyllis Joan Forbes (Leiter). He is survived by: his wife Cindy Fisher-Forbes (Hetzner) of Peru; his daughter Stacy Mavec (Michael) of Bloomington; his grandchildren, Grace Mavec of Bloomington and Michael A. Mavec, Jr. of Bloomington; his stepchildren, Melissa Ulshafer (Jim) of Urbana, Joe Fisher, Jr. of Mexico and Jeremy Fisher (Stephanie) of Roann; and his siblings, Leanne Voland (Francis "Bud") of Wabash and Rex Forbes (Joy) of Fort Wayne, Indiana.. He is also survived by 10 step grandchildren, 9 step-great-grandchildren.
