He was predeceased by: his father James Smith; and his brother Jamie Smith. He is survived by: his mother Connie Smith of Peru; his siblings, Kim Martin (John) of Peru, Vicki Worl (Kerry) of Bunker Hill and Bryan Eyman (Kelly) of Fort Wayne; and his pet Lucy. He is also survived by several nieces and nephews.
