He was predeceased by: his wife Emma Jean Hayworth (Scott); and his brother Dean Hayworth. He is survived by: his daughter Connie Shoemaker; his granddaughter Desiree Daniel; his great grandchild Olivia Daniel; and his siblings, Beverly Lank (Wally), Wayne Hayworth (Dixie) and Butch Hayworth (Pat Booker).

Memorial Contributions may be made to the Oxford United Methodist Church.
