He is survived by: his children, Jessica Everitt (David) of Scottsburg, Jeremiah Perry (Shawn Maier) of New Albany, Trent Woodward of Orleans and Damien Woodward of Salem; his stepdaughters, Stacey Jones of Salem and Sandra "Skeet" Apple of Salem; and his brothers, Doug of Salem, Phillip Perry of Salem and Greg Perry of Fredericksburg. He is also survived by great-grand children and three nieces.
