He was predeceased by: his parents, John Maddox and Susie Maddox (Hensley); his siblings, Oscar Hensley, Mary Lou Campbell and Gary Maddox; his grandchild Savanna; and his great grandchild Zayden Miller.
He is survived by: his wife Analiza Maddox (Camilo); his daughters, Christine Maddox (Anthony Schorle) and Anita Maddox (Patrick Sons); his brother Tommy Kent of Louisville, Kentucky; his grandchildren, Karlie Ritchie, Kameron, Patience Schorle, Kiara, E. J. Walls, Kendal, Nola and Shadyn; his sons, Zoey and Madison Miller; his great grandchildren, Savanna, Colton Warner, Gracie Kuhfahl, Graclyn, Rayah, Zaeliey Spaulding, Atlas Walls and Elliot Miller; his sister-in-law Janice Camilo of Louisville, Kentucky; and his close friends, Raymond Kidwell of North Vernon and Anna Yates of North Vernon.
Memorials may be made in honor of Jerry through the funeral home for expenses.
